The Viewer’s scale bar is located in the lower left corner of the screen, next to the Overlays Widget. It is only available in 2D mode, since scale varies across 3D images.
The scale bar updates its size and measurement units automatically as you zoom in or out on the screen.
If you prefer to see your measurements in different units, click on the scale bar to view a menu where you can select either imperial or metric units.
The scale bar is intended to give a general sense of scale only. For highly accurate measurements, use the 2D or 3D measuring tools.
